This page will guide you through what to expect during your first visit, including how to prepare, what to bring, and how we tailor your treatment to meet your individual needs.

Your first visit.

What to bring to your first appointment:

  • Prescription from your MD (if needed by insurance)

  • Insurance Card and Identification Card

  • Any Pertinent Medical Records (X-rays/MRI/Pet Scan Reports, Medical Records/Notes, Medication Lists)

  • Wear Loose and Comfortable Clothing for area being treated

  • Completed Patient Intake Forms

What to expect on your first visit:

The therapist will:

  • Review your past medical history and current health concerns (pain, triggers, relief).

  • Understand how your condition impacts daily activities and identify functional limitations.

  • Discuss your goals for therapy

The therapist will perform a physical exam, which may include:

  • Palpation, range of motion, muscle testing, neurological screening, special tests, and posture assessment.

Based on the evaluation, your therapist will:

  • Identify contributing factors to your condition.

  • Work with you to create a personalized treatment plan, including session frequency, duration, home exercises, and goals for progress and discharge.

Your treatment plan will be customized with input from both you and your therapist, ensuring it meets your unique needs.
